New CSD Student: Enrollment
Account Request
This form is the first step to enrolling your new student online. Complete it to request an account that you will use to log in to a secure system.
New Families Only (families that do not have any students currently enrolled in the district):
Complete the required fields in order to request an account to begin the enrollment application process for your new students.
Families with other students enrolled in the district (needing to enroll additional/new students to the district):
Log into your Skyward Family Access account and proceed with the enrollment process for new students in the left side bar titled “New CSD Student: Enrollment”.
When entering Address Information please follow these guidelines:
  1. House # – please use capital letters;  do not put spaces between sections ex: use N22W667, not N22 W667
  2. Street Name – If your street name pops up in the list while typing, please select it.  Do not create new street names with full suffixes to replace abbreviations.  ex:  Choose Washington ST  do not add Washington Street.
